About the event

Transitions and Legal Planning for Life After Age 18!

This session is most appropriate for parents, students, teachers, providers, and/or other professionals involved in transition planning at any age. Transition is the necessary link between school and the student’s future and their quality of life throughout adulthood.

Transition may involve a person’s right to self direction and may include assessing the need for legal planning to include the use of guardianship, conservators and/or Durable Powers of Attorney to enable the student and their families to realize their dreams and desires for the future.

This session covers the following areas:

Ø School Age Transition Planning

Ø Eligibility and Qualification for Social Security and Medicaid
